Salfordians invited to take control of their long-term health conditions


Salford residents who live with a long-term condition such as cancer, diabetes or kidney disease are invited to an information session on Tuesday 27 October 2015.

The event, which is organised by Salford Clinical Commissioning Group (CCG) aims to give Salford residents living with a long-term condition an understanding of the range of services available for them in the city to help them to live long, healthy lives. In addition there will also be opportunities for guests to join in some gentle exercise sessions, complete a personal wellbeing plan and also take part in a mindfulness session.

The event will be held at Buile Hill Park twice on the day, between 1.00 and 3.30pm and 5.00 and 7.30pm, and attendance is free for any Salford resident.

A new ‘Long-Term Conditions Locally Commissioned Service’ has been launched in Salford, which puts a minimum standard in place for GPs about what care they can offer. It is hoped that guests at this event will become more knowledgeable about what is on offer to them, so they can confidently manage their condition on their own terms, with the support network available to them in the city.

Dr Girish Patel, GP at The Sides Medical Centre in Swinton and clinical lead for long-term conditions at Salford CCG said:

“Living with a long-term condition can be a lonely and daunting prospect for many people.

“However, this need not be the case in Salford, as there is a vast amount of support available to people to enable them to take control of their condition and lead a long, happy and healthy life.

“This event will provide those living with such a condition with the information and tools that they need to take the necessary steps to confidently manage their health with the knowledge that there is a strong support network out there for them.”
