Salford Together – the city’s integrated care programme bringing health and social care together for adults – has made the shortlist for the prestigious Health Service Journal 2016 Awards.
The awards celebrate the best health projects in the country.
Health bosses launched the £213m budget Integrated Care Organisation (ICO) in July.
It now controls more than 2,000 staff across adult community, mental and acute health and social care services.
It’s hoped the ICO will help cut the number of hospital admissions and shorten the length of hospital stays.
Salford City Council’s lead member for adult services, health and wellbeing, Councillot Tracy Kelly, said combining adult social care with the NHS had been “a huge step” for the city.
“We believe this will deliver improved and more personalised services for residents in the face of reduced resources and increasing demand,” she said.
“By bringing contributions of social workers, GPs, district nurses, mental health professionals, care homes, voluntary organisations and local hospitals into a more joined-up system, we can help people stay at home longer and avoid unnecessary hospital admissions.”
Included in the ICO are Salford Royal, NHS Salford Clinical Commissioning Group, Salford City Council and Greater Manchester West Mental Health NHS Foundation Trust.
The winners will be announced in London on 23 November.
